We are currently accepting new patients ages 5-30. Before your first appointment, please complete and return all required intake forms, including permission to treat, health records authorization, and age-appropriate background information.

We accept most major commercial insurance plans, Medicaid plans and their MCOs, and private pay. We do not accept Medicare. Please notify our office of any insurance changes before your appointment.

Please have the following forms filled out and returned to us before making an appointment. You can submit your completed forms by email at admin@claytorclinic.com or by fax at 877-702-6432. 


  • Permission to treat
  • Authorization to Release Health Records
  • Child Psych Background Info, Adult Psych if over 18 years old
  • The last page of the Practice Parameter signed due to the parameters being something for you to keep

Please be aware that when insurance changes, most new insurances will require a prior authorization for medications. This means they will accept or not accept the medication being prescribed and/or pay for it. Please give us at least 3-5 days to get a prior authorization done for the medication.
